Fístula de palato e suas implicações na fissura labiopalatina em um estudo clínico randomizado (2015)

- Abstract: OBJECTIVE: The major goal of primary palatal repair is a successful reconstruction of the elevator muscle sling resulting in adequate velopharyngeal mechanism for adequate speech production and at the same time preventing complications on middle ear and avoiding facial growth impairment. The incidence of residual oro nasal fistulas is one of the important factors indicating success of primary palatal surgical treatment The literature about fistula results in a wide range of rates of occurrence, from 0% to > 60%. The definition of fistula in this study is a failure of healing or a breakdown of the primary surgical repair of palate according to Muzaffar et al., 2011. This study aims to contribute with informations about fistula occurrence after palatal primary repair in unilateral cleft lip and palate (UCLP) patients and to compare fistula occurrence between two techniques for palate repair Furlow (F) and von Langenbeck (L); to compare fistula occurrence among four surgeons who performed the palatoplasties; to compare fistula occurrence between two age groups at the time of palatal surgery: to verify the relation between fistula occurrence and relaxing incisions, considering the variables: surgeon and surgical technique of palate repair. MATERIAL AND METHODS: A retrospective analysis of medical records of 466 patients with unilateral cleft lip and palate (UCLP) without syndromes, included in a randomized clinical trial, operated by four surgeons using two techniques for the palate primary repair: Furlow and von Longenbeck and operated in two age groups: between 9 and 12 month (earlier surgery) or between 15 and 18 months (later surgery). RESULTS: Descriptive statistics revealed a fistula occurrence with Furlow technique of 20,47% at post-incisive foramen,while the rate was 11,32% for the von Langenbeck procedure. Among surgeons the rate of fistulae varied between 7,84% and 37% for the F technique and varied between 5,47% and 18,46% for von Langenbeck procedure. We concluded that fistula occurrence was higher with Furlow procedure than with von Langenbeck procedure and the occurrence varied among the four surgeons. Further understanding of the implications of fistula occurrence after primary palatoplasty requires more details regarding its implications on speech, food nasal regurgitation, management of fistula (surgical or prosthetic) and the co-existance of velopharyngeal dysfunction.
