Post extraction complications in patients with type 1 and type 2 diabetes (2022)

- Abstract: Hyperglycemia and diabetes mellitus (DM) increase the risk of infection and delayed healing after major surgery, but do not appear to affect healing after tooth extractions. Our aim was to verify and compare the frequency of complications after tooth extractions in patients with type 1 DM (DM1) and type 2 DM (DM2). In addition, we observed whether there is a relationship between postoperative complications and the prescription of prophylactic antibiotics. We collected data and retrospectively analyzed 311 medical records of patients with DM who underwent at least one tooth extraction, totaling 68 (21.9%) patients with DM1 and 243 (78.1%) with DM2. In the 311 patients, 895 extraction interventions were performed; 192 extractions occurred in patients with DM1 and 703 in patients with DM2. Patients with DM1 showed 09 (4.7%) records of complications after extractions, namely: pain (1/192), bone spicule sequestration (3/192), alveolitis (2/192), delayed epithelialization (2/192) and abscess (1/192). In patients with DM2, we computed 14 (2%) records of complications: pain (7/703), bone spicule sequestration (3/703), alveolitis (2/703), abscess (1/703) and paresthesia (1/703). Antibiotics were prescribed for 26% of patients with DM1 (50/192) and for 15% of patients with DM2 (105/703). No association was observed between postoperative complications and prophylactic antibiotic prescription. The incidence of complications after tooth extraction was low and similar between patients with DM1 and DM2
