Análise molecular e perfil de susceptibilidade às drogas antifúngicas (2010)

- Abstract: Candidíase mucocutânea crônica (CMC) é uma rara e complexa síndrome que afeta um determinado grupo de pacientes. Essa patologia caracteriza-se por apresentar uma infecção persistente ou recorrente na pele, unha e nas membranas das mucosas, sendo causada por organismos do gênero Candida principalmente Candida albicans. Essa síndrome é caracterizada por apresentar uma alta morbidade e mortalidade, sendo sua causa atribuída principalmente a desordens no sistema imunológico, contudo, muitas dúvidas persistem sobre a sua causa. Em nosso trabalho estudamos 9 cepas isoladas de pacientes com CMC, onde avaliamos a susceptibilidade aos antifúngicos anfotericina B, fluconazol, voriconazol empregando três metodologias; microdiluição pelo método preconizado pelo documento CLSI, M27-A3, documento EUCAST e método comercial E-Test®. Também analisamos molecularmente os isolados, buscando conhecer aspectos relacionados aos fungos. Nossos resultados mostraram que todos os isolados foram Candida albicans de acordo com a técnica de PCR empregada. Com relação ao perfil de susceptibilidade observamos que para a droga anfotericina-B e voriconazol todos os isolados foram sensíveis, entretanto para a droga fluconazol apenas 1 isolado apresentou resistência e 1 isolado foi S-DD (Sensível-Dose Dependente), de acordo com o método de referência M27-A3. Comparando as metodologias observamos que somente para o método comercial E-Test® houve discordância entre os resultados, pois 2 isolados mostraram-se resistentes para a droga fluconazol. Nossos dados mostraram que Candida albicans continua sendo o agente causador dos casos de CMC estudados. Com relação ao perfil de susceptibilidade às drogas antifúngicas observamos que dois isolados apresentaram baixa susceptibilidade à droga fluconazol.
