- Abstract: Neurothekeoma, first described in 1969 by Harkin and Reed as a rare neoplasm of peripheral nerves, which is characterized as a benign lesion. Clinically it is presented as a rare cutaneous lesion, with few described cases in the oral mucosa. This work reports a case of cellular neurothekeoma located on the floor of the mouth, the first at this site. The lesion was present in a 60- year-old female patient, leucoderma, as a nodule pedunculated with approximately 10mm of diameter, for 9 months. The diagnostic hypothesis was of related traumatic injury to the use of removable prosthesis. Excisional biopsy was performed and it was sent for morphologic analysis. The microscopy showed oral mucosa constituted of hyperplasic stratified squamous epithelium with parakeratinization. Underlying in the submucosa, proliferation of spindle rounded cells with large and vacuolated nuclei in a collagenous stroma and focal areas of myxomatous stroma with small cells with star format were observed. Fibrils with storiform standard were also noticed. To the immunohistochemistry it was observed that the neoplastics cells had expressed positive marking for NKI-C3 and S-100, denoting neural histogenesis of the lesion. Cellular Neurothekeoma is an unusual benign neoplasm which, despite its name, is of uncertain origin. It was, however, completely excised and in a 6 months follow up, no recurrence was observed
