  • Abstract: Introduction: COVID-19 infection caused a global pandemic and was responsible for more than 6 million deaths and more than 600 million cases across the globe. In addition to pulmonary complications, thrombotic manifestations such as acute limb ischemia (ALI) and mainly venous thromboembolism (VTE) were frequently seen in these patients. The literature shows an incidence of up to 26% of thromboembolic events in those infected with SARSCoV-2. Such events are directly related to the severity of the infection and higher mortality in these patients. There is a lack and divergence of evidence-based guidance for physicians dealing with these complications. Objectives: Analyze data from patients who required hospitalization at the Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (HCFMRP-USP) between March 20 and December 21, and had confirmed covid-19 infection and developed OAA and VTE. Compare epidemiological data and the management of these patients, as well as related outcomes, enabling a better understanding of the thrombogenic pathogenicity of this disease, as well as enabling institutional reassessment, which will help guide physicians towards the development of a best practice management approach in this scenario. Methods: Observational and descriptive study, with data collection from patients hospitalized from March 2020 to December 2021 at the Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (HCFMRP-USP) - Campus Unit and Urgency and Emergency Unit, witha diagnosis of covid-19 confirmed by RT-PCR and who have presented any of the following events: ALI and/or VTE. Demographic data, chronology of events, length of stay and severity of the patient's health condition, among others, were evaluated. Vascular complications, their concomitant occurrence and the treatment instituted were analyzed, including variables that impacted on decision-making, allowing the evaluation of outcomes during hospitalization and 90 days after the thrombotic event, such as death, major and minor bleeding, need for surgical intervention, among others. Results: The prevalence of VTE was 6.97% and the majority had pulmonary thromboembolism (92%); the prevalence of ALI was 0.9% (23). Most were men and white. The most prevalent comorbidity was arterial hypertension. Almost all patients were using anticoagulants before the thrombotic event, with enoxaparin 40 mg SC being the most used. Mortality during hospitalization of patients with ALI and VTE was 56.5% and 34.5%, respectively. Approximately 52.2% of patients with ALI were evaluated within 6 hours of the suspicion of the condition and 50% had resolution of the condition after targeted treatment. Conclusion: The population of the present study has demographic characteristics similar to those found in the literature. There was an increase in D-dimer in 61.4% of patients with VTE who had more than one measurement and in 50% of patients with ALI. Based on the findings of this paper, an institutional protocol was created aiming at a correct and earlier diagnosis of thrombotic events
