- Abstract: Introdução: Esse estudo teve como objetivo avaliar em longo prazo o impacto do tratamento ortodôntico com e sem extrações dentárias em tecido mole e estruturas dentoesqueléticas em perfil. Métodos: A amostra foi composta por radiografias cefalométricas de quarenta pacientes tratados ortodonticamente com e sem extrações de pré-molares. Os pacientes foram avaliados nos tempos pré-tratamento (T1), pós tratamento (T2) e acompanhamento em longo prazo (T3). Dois grupos foram formados de acordo com o protocolo de tratamento. O Grupo com Extrações: 24 pacientes (16 feminino, 8 masculino), com idades médias de 13,14 (T1), 15,50 (T2) e 52,49 anos (T3). Tempo médio de tratamento (T2-T1) e acompanhamento em longo prazo (T3- T2) de 2,36 e 36,99 anos, respetivamente. O Grupo Sem Extrações: 16 pacientes (10 feminino, 6 masculino), com idades médias de 15,18 (T1), 17,03 (T2) e 53,61 anos (T3). O tempo médio de tratamento (T2-T1) e de acompanhamento em longo prazo (T3-T2) foi de 1,85 e 36,58 anos, respetivamente. A análise cefalométrica dos perfis de tecidos moles e dentoesquelética foi realizada com o software Dolphin Imaging. O teste t independente foi utilizado para comparações intergrupos. Resultados: O grupo sem extrações teve um tempo de tratamento menor (p=0,31). O grupo com extrações apresentou retração significante dos incisivos inferiores durante o tratamento (p=0,019). Ambos os grupos apresentaram alterações semelhantes nas variáveis de tecido mole e dentoesqueléticas após o acompanhamento em longo prazo. Conclusões: O tratamento ortodôntico com e sem extrações apresentaram mudanças semelhantes em tecido mole e dentoesquelética de perfil após um acompanhamento de 37 anos.
