Cyclosporine and chronic irritative factors as combined agents in the etiopathogenesis of gingival hyperplasia: case report (2021)

- Abstract: This paper reports the clinical case of a 25-year-old male patient that presented to the dentistry division complaining of significant gum bleeding episodes while brushing the teeth. He was previously diagnosed with aplastic anemia and was under immunosuppression therapy with cyclosporine. On clinical examination, poor oral hygiene, gingivitis, and extensive fibrotic areas compatible with gingival hyperplasia were noticed. Oral hygiene orientation was performed along with chlorhexidine digluconate 0,12% prescription. Scaling and root planning sessions were carried out every 2 weeks, under antibiotic therapy with amoxicillin 500mg, every 8h, beginning 48h before intervention. In two months, hyperplastic areas diminished, and no gingival bleeding was detected. Chronic irritative factors, as the presence of bacterial plaque, are related to the etiopathogenesis of gingival hyperplasia; furthermore, it is well-established that immunosuppressors are also responsible for abnormal overgrowth of gingival tissue. Studies describe that Cyclosporine increases interleukin 6 secretion, stimulating gingival fibroblasts to produce more collagen and glycosaminoglycans. Patients using such medications need regular dental follow-up, intending to promote adequate oral hygiene measures to inhibit such occurrences, as well as implement appropriate protocols in the presence of lesions. Accordingly, the dental surgeon must be integrated into the multidisciplinary healthcare team, contributing to the control of morbidities in medically complex patients
