Placebo effect on painful temporomandibular disorders: a meta-analysis (2017)

- Abstract: The aim of this systematic review was to answer the following question: "In adult patients with painful temporomandibular disorders (TMD), what is the magnitude of the placebo or nocebo effect of pain therapies?" Two reviewers searched for randomized clinical placebo-controlled trials on diagnosed painful TMD, published on Cochrane, LILACS, PubMed, Scopus and Web of Science. A grey literature search was also performed on Google Scholar, Open Grey and ProQuest without sex, time of publication, or language restrictions. Pain relief measure on a Visual Analogue Scale (VAS 0-100mm) was the primary outcome. The methodology of selected studies was evaluated using the Cochrane Risk of Bias tool. Of the 473 articles obtained, 33 met the inclusion criteria for qualitative, and 21 for quantitative analyses. The meta-analysis found substantial pain improvement among active therapy patients: laser therapy with 76mm points reduction followed by laser acupuncture, medicines such as amitriptyline 25 mg or cyclobenzaprine, and splints with 37mm variation. Among placebo groups, greater pain reduction was found on laser acupuncture with 45.5mm points reduction, followed by avocado soybean extract with 36mm and medicine such as amitriptyline 25mg with 17mm. Nocebo effect was only observed for intra-articular injection of Ultracain. Based on the available evidence, the conclusion of this meta-analysis was that placebo effect is a substantial part on TMD pain reduction and may be responsible for about from 10 to 60% pain reduction depending on different therapies
