Oral expressed breast milk for pain relief in neonate: a literature review (2010)

- Abstract: Background: The effectiveness of expressed breast milk (EBM), for neonatal pain relief during minor procedures is controversial.Objective: To review published on the effectiveness of EBM as an analgesic strategy for neonates.Methods: Randomized, quasi-randomized and cross-over trials were retrieved from manual and electronic searches of MEDLINE, EMBASE, CINAHL, and PsycINFO.Results: From 694 titles identified, seven papers were included (726 neonates). Six studies focused on term neonates and one investigated term and preterm infants. Procedures studied were heel lance (n=6) and venipuncture (n=1). EBM was compared to sucrose, glucose, artificial sweetener, glycine, water, sucking, rocking, and massage. The combination of EBM and pacifier was used in one study. Outcomes included crying (n=7), changes in heart rate (n=7), grimacing or Neonatal Facial Coding System scores (n=4), changes in oxygen saturation (n=3), and Douleur Aigue Nouveau-né scores and Body Pain Score (one study each). When compared to sucrose (12-25%), glucose (25-50%), and artificial sweetener, results do not favor EBM in terms of crying, grimacing and physiological indicators. There were no differences in crying, grimacing and changes in heart rate between EBM and water. Rocking and sucking were more effective inreducing pain scores and duration of cry than EBM. At lower concentrations of glucose (10%), compared to EBM, there were no differences in cry duration (n=2 studies), nor heart rate and oxygen saturation changes (n=1 study). The use of a larger volume (5ml) of EBM resulted in shorter duration of cry, lower increase in heart rate and decrease in oxygen saturation compared to water.Conclusion: Although EBM is safe, free and easily obtained, the limited number of studies and inconsistent results preclude estabishing its effectiveness for pain relief. Further investigation is required to clarify EBM's role in neonatal pain management during minor procedures. However, providing adequate strategies for pain control is fundamental and existing evidence supports the use of sweet solutions as effective strategies for neonatal analgesia.
