- Abstract: Background: In this study, it is important to remember that AIDS metaphors such as death and horror contribute to stigma and discrimination. Methods: We analyzed in-depth interviews with 12 health care professionals from three health care services specialized in HIV/AIDS and 9 orphan caretakers. They were asked to imagine the future of orphans at 15 and 24 years old. Results: The difficulties in common imagined by interviewees refer to: adolescence issues in general (dating, sexuality, and reproduction) and HIV/AIDS-related (disclosure to partners, friends and coworkers; fear of re-infection or infection of partners, social isolation and impossibility of constructing a family). Also, they imagined additional difficulties or had trouble imagining the child\2019s future (even in the case of a seronegative orphan) when it occurred to them the deaths of elderly or HIV positive caretakers first. Regarding seropositive girls, interviewees imagined them at 24 without marriage plans and living with their caretakers. From the professionals\2019 perspective, it was difficult to imagine the future of AIDS orphans and death in the case of those who were HIV positive and currently in critical condition was predicted. Their future was only imagined with hope for a cure. Additional difficulties mentioned were: finding work due to the side effects of medicines damaging their physical appearance, and school work compromised due to constantly be interned in the hospital. Concerning institutionalized orphans, abandonment by their family and less love and affection were also expressed. Conversely, none of the caretakers mentioned the orphans\2019 deaths, even in the cases considered critical by the professionals. They did not imagine difficulties with work or study. Conclusions: Being seropositive produces unnecessary restrictions on the future of AIDS orphans, reinforcingnegative AIDS metaphors. The fight against stigma associated with AIDS must prioritize the care and protection of AIDS orphans\2019 rights.
