Frequency of dendritic cells and macrophages in the tumor microenvironment of lung cancer patients (2006)

- Abstract: Introduction and Objectives - Dendritic cells (DCs) play a crucial role in presenting antigens (Ags) to T lymphocytes. Although dysfunction of DC in malignancy is believed to occur, the underlying mechanisms are poorly defined and appear to be heterogeneous. However, their maturation has been shown to be impaired in cancer patients. To date, there have been few studies in patients with lung cancer. Thus, the aim of this study was to analyze the frequency of DC (both plasmacytoid and myeloid) and macrophage-marker-positive cells in the tumor microenvironment of patients with primary lung neoplasia. Methods and Results - Patients were analyzed as to the frequency of HLA-DR, CCR7, CD11c, CD14, CD40, CD80, CD83, CD86 and CD123 positive cells in the primary tumor and in non-affected lung tissue by flow cytometry. No consistently significant differences were observed between these tissues, though in individual patients they were present. The average percentage of positive cells for the markers, for tumor and non-affected lung respectively, were: HLA-DR+ (37.19% and 34.95% p=0.65), CCR7+ (17.45% and 24.44% p=0.2), CD11c+ (28.34% and 27.37%p=0.97), CD83+ (58.54% and 50.54% p=0.74) CD40+ (3.65% and 1.8% p=0.34), CD80+ (6.58% and 9.4% p=0.7) and CD86+ (4.42% and 3.66% p=0.3).This low frequency of CD40, CD80 and CD86+ cells is intriguing due to the high frequency of HLA-DR, CCR7 and CD83+ cells. Likewise, there were no differences betweentissues in the frequency of plasmacytoid DC (CD11c- CD14- CD123+; 5.27% and 6.44% p=0.6) and myeloid DC (CD11c+ CD14- CD123-; 14.15% and 13.66% p=0.92) and macrophages (CD14+; 14.72% and 13.83% p=0.7). Conclusions - Our data suggest that the DCs, if present within the tumor may be poor Ag-presenting cells due the lack of co-stimulatory molecules, a phenotype that would be expected in non-affected lung tissue, but not in the tumor, if an immune response against it would be to occur. Furthermore, the high frequency of CCR7+ cells may also indicate the migratory capacity of such cells, whose inability to co-stimulate T cells, might be involved in tolerance induction to tumor Ags.
