Mulberry-like ulceration: case repor (2006)

- Abstract: A 40 year-old white man, smoker, complained about "inflammation in the mouth". Five months ago, after dental extractions, there was emergence of ulcerations. Extraoral physical exam revealed swelling of the 1/3 left inferior of the face, close to the left labial commissure; one mobile, hard and painful submandibular node, and macroqueilia. Intraoral physical exam could be observed extensive mulberry-like ulceration areas, in the labial commissurae, in the superior lip and in the alveolar ridges. The presumptive diagnoses were paracoccidioidomycosis and squamous cell carcinoma. Incisional biopsy by punch was made in the inferior anterior alveolar ridge. The microscopic exam revealed fungal sporulation, bluish-colored nuclei, birefringents capsules surrounded by a granulomatous process. An AP thorax radiograph revealed lung image with aspect of butterfly wing. The microscopic features proved the first diagnosis hypothesis. The treatment was based on antifungal drugs (Anfotericin B (I.V.), Itraconazol), nutritional complementation, smoke and alcohol restriction; being respected the therapeutic outline of two phases: attack and maintenance
