Infected lesion in the jaw (2006)

- Abstract: Patient melanoderma, 53 years, was attended with "purulent secretion in the gingiva" and historical of extraction in the area 8 months ago. To the inspection it presented a little swelling in the left side of the mandibula and a small fistula draining pus. The panoramic radiograph revealed a lesion of mixed radiopacity in the region of inferior right molars, measuring 2 cm of diameter; another radiopaque lesion in the area of left premolars, with 6mm of diameter - both well delimited; and other radiolucent lesion with central radiopacity and radiopaque halo, measuring 1,5 cm of diameter. The presuntive diagnosis was infected cemento osseous dysplasia, infected periapical cyst in association with cemento osseous dysplasia and osteomyelitis. After antibiotic therapy, were accomplished biopsy and curettage of the area. Microscopic exam presented areas calcified in trabecular form, presence of bacterial biofilm, hemorrhagic content and necrotic remains. In the granulation fabric removed was observed intense inflammatory infiltrated and areas of calcification. The microscopic characteristics add to the clinical and radiographic findings was compatible with the diagnosis of secondary suppurative osteomyelitis in the fibro-osseous lesion, suggestive of cemento osseous dysplasia
