Effect of bay 41-2272 in the pulmonary hypertension induced by heparinprotamine complex in anaesthetized dogs (2005)

- Abstract: Objectives: The in vivo heparin-protamine interaction leads to marked cardiovascular disturbances in humans, including pulmonary hypertension (PH). We have studied the effects of BAY 41-2272, an activator of NO-independent site of soluble guanylate cyclase, in the PH induced by heparin-protamine interaction in anesthetized dogs. Methods and Resulted: Ten male mongrel (8-15 Kg) and 6 male Beagle dogs (8-12 kg) were anaesthetized with pentobarbital sodium (30 mg/kg, i.v), and supplemented with fentanyl (10 µg/kg/h) and diazepam (700 µg/kg/h). The animals were injected with heparin (500 IU/kg, i.v., bolus) followed 3 min later by protamine (10 mg/kg, bolus, i.v.). The following parameters were evaluated: blood pressure (BP), heart rate (HR), PH, cardiac index (CI), index of systemic and pulmonar vascular resistance (ISVR), (IPVR). The heparin-protamine complex caused a marked PH, which elevated from 9.8±2.3 to 31.2±4.6 mmHg at 2 min after protamine injection (P<0.01). This was accompanied by a BP decrease (from 94±10 to 60±15 mmHg), and increases in both HR (106±9 to 160±2 beats/min) and IPVR (246±55 to 1588±448 D.s/cm5/m2). Bay 41-2272 (10 µg/kg/min, i.v.) markedly prevented PH and the increased IPVR, without significantly affecting BP and HR. Conclusions: Our results suggest prolonged stimulation of sGC in a NO-independent manner by BAY-412272 counteracts the vasoconstrictor mechanisms at the pulmonary bed that triggers the PH. BAY- 412272 mayconstitute a promising drug for treatment of PH
