Hypericum perforatum and fluoxetine augmetns 6-sulphatoxymelatonin concentration in patients with major depression (2003)

- Abstract: Objectives: The aim of the study was to establish if there is a change in the 6-sulfatoxymelatonin (6smel) secretion profile of patients with major depression submitted to antidepressant treatment and the relationship of this profile with clinical condition. Methods: 43 patients with major depression took part in an 8-week double-blind placebo controlled trial, receiving Hypericum perforatum 500mg or fluoxetine 20mg. 6smel concentration in total 24-h urine and divided by 6 hours intervals was determined. Creatinine was also measured in urine as an appropriate reference for 6-smel concentration (Klante, G., J Pineal Research 23: 191, 1997). Patients´ clinical condition was evaluated through depression rating scales (Hamilton's, HAMD; and Montgomery-Asberg's, MADRS), and the Clinical Global Impression scale (items severity and improvement).The 6smel in morning urine was significantly increased at the end of treatment with fluoxetine (7.52 + 2.24 vs 36.6 + 6.03) though it did not change the total 24-h 6smel secretion. Hypericum perforatum was associated with significant increase of 6smel in the night (14.7 + 4.6 vs 33.5 + 2.4) and in the total 24-h secretion (29.2 + 10.8 vs 77.5 + 14.9). *=P<0.05. The increase in 6smel concentration was only observed for patients whose clinical condition improved with treatment. Conclusion: 6smel secretion has been considered a physiological index for noradrenergic function, but we are here showing for the firsttime that it reflects a marker of depression improvement after treatment with drugs that inhibits noradrenaline or serotonin uptake
