Perceptual and instrumental evaluation of velopharyngeal function in patients with asymptomatic submucous cleft palate (2001)

- Abstract: The objective of the present study was to evaluated velopharyngeal functioning individuais with submucous deft palate (SMCP) not associated with speech symptoms caused by velopharyngeal inadequacy (VPI); the intention was to assess whether patients considered to be asymptormatic on a perceptual speech evasliation indeed had adequate velopharyngeal closure and absence of hypernasality when evaluated instrumentally. We studied 20 individuals with SMCP of both sexes aged 6 to 46 years without symptoms of VPI upon perceptual speech evaluation. All subjects were also submitted to acoustic evaluation (nasometty) using a Nasometer 6200 (Kay Elemetrics) for the determination of nasalance in the production of sentences exclusively containing oral sounds, and to aerodynamic evaluation for the determination of the velopharyngeal orifice area in the production of the plosive sound /p/ inserted in the word "rampa" (pressure-flow technique) using a Perci-Sars System (Microtronics). The results of aerodynamic evaluation confirmed the observation of perceptual evaluation, i.e., the 20 (100%) patients considered asyntomatic presentes adequate velopharyngeal dosure (area <5mm2) upon instrumental evaluation. On the other hand, the results of nasometric evaluation did not confirm those of perceptual evaluation in 25% of cases, i.e., 5 of the 20 patients considered asynptomatic (with no resonance deviations) presented nasalance scores suggestive of hypemasality (nasalance >27%) upon instrumental evaluation. The results show the importance of the combined use of perceptual and instrumental evaluation in the diagnosis of VPI in patients with SMPC. We recommend periodic follow-up of cases considered asymptomatic upon perceptual evaluation and presenting evidence of VPI upon instrumental evaluation, especially in the case of children, who are more prone to the development of symptoms with advancing age
