Study of nasal mudus in patients with cleft lip and palate (2001)

- Abstract: Patients with deft lip and palate (CLP) present a high incidence of upper airway infections. The observation of a decreased in vivo mucociliary clearance suggests that this is a mechanism involved in the genesis of upper airway infections. Changes in mucociliary transport can be related to alterations in the cilia, mucus or its interaction. The objective of this study was to compare the nasal mucus transportability and physical properties of patients with CLP to the mucus of normal subjects. The mucus of 31 patients with CLP and 16 of normal subjects was collected by nasal aspiration. In vitro mucus transportability (MT) by cilia was evaluated by the method of the frog palate (results are expressed as relative speed). Cough clearence (CC) was evaluated by the method of simulated cough machine (results are expressed in mm). Mucus physical properties were evaluated by the contact angle (CA), by measuring the angle (in degrees) between the mucus sample and a glass surface. Mucus rheologic properties were determined by means of a magnetic rheometer, and the results are expressed as log G' (vectorial sum of viscosity and elasticity) and Tan del (relationship between viscosity and elasticityl measured at 1 and 1 00 rad/s. The results for the normal and CLP groups, expressed as mean ± standard deviation (*, p< 0,05) were, respectively: MT= 0.87±0.29 and 0.85+0.39; CC(mm)= 41±24 and 65±32'; CA(degrees)= 35±11 and 24±15*; 10g G'I= 1.43±0.4 and 1.52±0.71; Tan dell- 0.42±0.1 and 0.49±0.24; 10g G'JOO= 1.52±0.28 and 1.53±0.S8; Tan dell 100= 0.51±0.12 and 0.72±0.23'. The results suggest that patients with CLP have a higher transportability by cough when compared to normal subjects. The inccreased number of upper airway infections in these patients cannot be explained by changes in mucus physical properties and transportability.
