Inter-relation between hipomobility and hipermobility of the temporomandibular joint (2006)

- Abstract: According to the classification proposed by the Academy of Orofacial Pain, the disorders of the temporomandibular joint (TMJ) may cause restricted (hipomobility) or widening (hipermobility) mouth opening. Among the disorders of the temporomandibular joint that may cause hipomobility or closed-lock, there are the disc displacement disorders, like the disc displacement without reduction, which is characterized by the displacement of the disc on closing followed by a failure to reduce or recapture the disc during translation. The hipermobility is classified as a displacement of the disc-condyle complex, meaning the movement of the complex forward as a unit beyond the articular crest. These antagonic clinical conditions are usually presented in an isolated form. However, one-side articular hipermobility may occur as a compensatory reaction to the hipomobility on the contra-lateral joint (Bell, Goldman). On this clinical case report, a 23 year-old woman was seen for treatment with pain, clicking and closed-lock on joints. After a clinical exam, muscle palpation and radiographic exams, the coexistence of the chronic disc displacement without reduction on the right TMJ and hipermobility was diagnosed. The treatment employed has consisted on the use of a repositioning oclusal splint and exercises for hipomobility and hipermobility
