Brazilian large scale acuity card study: 2 the screening study (1994)

- Abstract: Purpose: To eavaluate the validity of Teller Acuity CArds (TAC) as a screening tool for early detection of visual disorders using ophthalmologic and orthoptic exams as a gold standard. Methods: TAC was done binocularly and monocularly in 73 children form 0 to 36 months old, recruited mainly in day-care centers. Of these, 688 received ophthalmologic and orthoptic evaluations. Criteria for referral were: 1 Significant refrative errors, anisometropia, ocular motility disorders, ocular diseases and/or abnormal external aspect of the eyes; 2. VA octave below age norms (Salomaâo & Ventura, IOVS, 34 (4): 1420, 1993) and/or interocular acuity difference (IAD) octave Results: Testability was 97,5% and average test durationwas 13 min. Visual disorders were detected in 45/688 (6.5%) children. Abnormal ocular findings with normal VAs were: a. ocular diseases: 1 bilateral puctiform cataract, 1 bilateral lachrymal obstruction and 1 bilateral congenital glaucoma (the latter not confirmed after referral); b. ocular motility disorders: 10 cases - 2 esotropes, 3 exotropes, 5 intermittent exotropes; e refractive errors: 9 cases (all bilatral). Abnormal ocular findings with abnormal VAs a. 1 case of monofixation syndrome with anisometropic ambylopia, confirmed by follow-up; 1 esotrope with an IAD of 3.5 octaves, and 1 bilateral hyperopic. Abnormal VAs with normal ocular findings: 25 children (2 with VA below age norms, 20 with IAD 1 octave and 3 due to both reasons). Validity , calculated excluding the 5 intermittent extropes, resulted in sensitivity of 18% and specificity od 97%. Conclusions: Although TAC has low sensitivity when comapred with the avalilable gold stadard, its use in screening along with other isntruments is recommended since it can detect visual acuity deficit, as confirmed by follow-up
